What a prescription layout contains
A medical prescription pad layout usually shows the clinic or doctor details at the top (name, credentials, address, logo), then patient fields (name, age, gender, date), a diagnosis or Rx area, a list of medications with dosage, frequency, duration, and instructions, and a signature block. Printers, educators, and product designers reuse that same visual order so trainees and test users recognise the document at a glance. Using a generated prescription to obtain medicines, or presenting it as medical proof, is illegal in most countries. Only a licensed clinician issuing a real prescription through lawful channels can authorize medications. Keep these files for design, teaching, testing, and props — never for pharmacy fulfilment or clinical records.
